Deploy Azure Application
EZCARE is deployed directly into your Azure environment as an Azure Managed Application. T2 handles all application configuration, updates, monitoring, and maintenance. Your team simply provisions the required Azure access, allowing for a secure, streamlined deployment with minimal IT effort.
EHR Integration
EZCARE receives visit summaries through a turnkey vendor that is already integrated with most major health systems. Implementation is straightforward, once the necessary BAAs are executed, the health system simply enables the API, allowing visit summaries to flow seamlessly into EZCARE.
Patient Portal Activation
We connect EZCARE to MyChart IDP and work with Epic to add the patient notification hook.
Fulfillment Routing
Third-party fulfillment partners are activated to handle logistics, inventory, and delivery, automatically routed from patient orders.
Go Live and Revenue
EZCARE goes live and tracks care plan compliance, providing valuable post-visit insights, analytics, and new revenue.
